We are so excited to bring you this awesome news! If you have a "Renew Active", "One Pass", or "Aaptiv" plan with United Healthcare,  you MAY be eligible for no-fee or a reduced  price!!

How do you know if you're eligible? If you have United Healthcare and you know your Review Active,  One Pass, or Aaptiv "confirmation code", that is all we need!. Contact us with this code and we will check your eligibility.

• For Medicare/Medicaid memberships, it will start with an "A", "S" or "G" and will be followed by 9 digits. 
• For Commercial memberships through your employer, it will start with a "B" or "P" and will be followed by 9 digits
• If you don't know this code, please send the following information to one of our two centers (gulfbreeze@osteostrong.me OR cordova.pensacola@osteostrong.me) and we will confirm if you are eligible. 

1. Name (exactly as it reads on your insurance card)
2. Membership ID number
3. Group number (if it's listed on card)
4. Birth date 
5. Zip Code

Some important things to know:

• Not all United Healthcare plans have the Renew Active / One Pass benefit.
• The United Healthcare partnership ONLY covers the 4 "Spectrum" axial loading machines.  A fee is required to add additional innovative wellness solutions (vibeplate, biocharger, PEMF, redlight, Normatech compression boots, and hydromassage).
• If you are already on a discounted plan (legacy or military) this could mean that your current rate is better than what you would pay out of pocket through this plan.  We will be happy to confirm the pricing difference for you.
• Current members who decide to apply the OPTUM plan to supplement their current OsteoStrong membership forfeit any current discounts.
• The BEST news is this is just the beginning - most likely this means that in the future, more insurance companies and more plans offer coverage. We'll keep you updated on any developments in that arena!
